• Please familiarize yourself with our Business Terms and Conditions. 
• Complete our Customer Information Form.
• All accounts wishing to charge purchases must complete and mail or fax in our standard Credit Application form. We also accept prepayment by check, Visa or Master-card. Because we ship common carrier for greater speed, we generally do not ship COD.
• Complete our Online Customer Information Form to sign up for Foster's Online Account Access. You must also fill out the Customer Information Form above to receive Onlince Account Access.
